Summary

Physio Pal is registered with the NDIS to provide Physiotherapy Supports and Exercise Physiology Supports (Therapeutic Supports)

Our invoices will show the following code from Support Category 3.15:
- 15_055_0128_1_3 (Assessment, Recommendation, Therapy, and/or Training – Physiotherapy)
- 15_200_0126_1_3 NDIS Exercise Physiology

Hourly rate:
At the time of this agreement, our fees are:

- Individual Therapy Services: $193.99 per hour
- Exercise Physiology: $166.99 per hour
- Therapy Assistant – Level 1: $56.16 per hour
- Therapy Assistant – Level 2: $86.79
- Group Therapy – Group of 3 – Physiotherapy: $64.66

Travel:
*Please note we aim to group clients together in similar areas to minimise travel costs.
- The maximum we can claim for travel per appointment is 60 minutes ($193.99).
- This consists 30 minutes TO you, and up to 30 minutes return, if you are the last appointment of the day.
- We ask your permission to apportion travel between participants. This means we our travel costs for a day are shared equally between participants, with the aim of minimising your travel fees whilst covering the cost of our time in delivering a quality mobile service.

GST:
- Most services provided under the NDIS do not include GST.
- Physio Pal does not apply GST to any claims from Support Category 3.15: Improved Daily Living Skills.
- A supply of supports under this Service Agreement is a supply of one or more of the reasonable and necessary supports under subsection 33(2) of the National Disability Insurance Scheme Act 2013 (NDIS Act), in the participant’s NDIS plan currently in effect under section 37 of the NDIS Act.


Participant Responsibilities
- Cancellation policy: we ask that all participants provide us with sufficient time prior to cancelling an appointment with as much notice as possible.
- Please inform us prior to 3pm the day before our scheduled appointment.
- Costs: if cancellations are made after this time, we will charge a 75% fee per unit cost of the scheduled appointment.
- Exceptions: if the cancellation reason is unforeseeable, or due to a medical condition the cancellation fee will be waived.

Provider Responsibilities
- Work with the participant with a patient centred approach to achieve their NDIS plan goals and include them on all decision making for planning of the agreement and all supports provided within that agreement.
- Provide adequate communication regarding times and locations of appointments to reduce the risk of late changes to the delivery of supports.
- Provide easy to access contact information to allow sufficient time for cancellation, feedback or service agreement changes.


Conflict Resolution/Service Agreement Adjustments

If an issue arises during the period of the service agreement, please inform us as soon as possible. We will work to solve the issue together to continue to provide quality delivery of services. Please see your information pack on how to provide feedback or make a complaint. We are always striving to deliver quality care and implement ways to respond to your feedback and needs.

Changing this agreement

We can make changes to the dates, supports, and/or funding contained in this agreement. These changes must be made in writing (e.g. email or letter). All changes must be agreed upon by both parties.

Termination of Service Agreement

In the event that you would like to terminate the service agreement, please inform us in writing either by email or mail, with a notice period of one (1) month.
